Role Description
While embracing the mission and vision of the Diocese of Green Bay, the Family Resource Coordinator is responsible for helping clients and their families to navigate and access community services and adopt healthy behaviors. The priorities are to promote, maintain, and improve the holistic health (physical, emotional, spiritual) of clients and their family. The Family Resource Coordinator provides social support, informal counseling, advocacy for clients with providers/community supports, outreach, and provides short term case management services. In addition, the Family Resource Coordinator will be responsible for fulfilling all contractual requirements for grant funding and meeting outcome goals.
Qualifications
- Required associate degree or equivalent based on relevant work experienced required.
- Bilingual or multi-lingual preferred (written and oral proficiency)
- Preferred: prior experience working in a multi-cultural setting and/or holds community health worker certification
- The ideal candidate will have a bachelor’s degree and knowledge in social work, human development, community services, psychology, human services, community health, or related field.
- Experience working in a community-based setting for at least 1 or 2 years preferred.
- Proficient in Microsoft Office products (Word, Excel, PowerPoint, Teams, Outlook)
- Excellent writing skills and interpersonal skills
- Strong communication skills
- Demonstrated ability to work in a team environment.
- Ability and willingness to provide emotional support, encouragement, and motivation to clients in order for clients to navigate systems independently.
- Skilled in problem solving and finding creative solutions to challenges.
- Ability to work with other systems without intimidation.
- Must be able to organize and prioritize work, take initiative, follow through and manage multiple priorities to ensure work and goals are met in a timely manner.
