JOB SUMMARY: The Specialist is the primary advocate for adults with mental illness & chronic/serious medical diagnoses. These consumers will be referred by the Division of Mental Health Services & from within our own continuum of housing services to establish independent living situations which provide specialized supports. The Specialist must coordinate each individual's services with the rest of the MESH Team & participate in the direct provision of these services.
DUTIES & RESPONSIBILITIES:
- Responsible for ensuring that all needed medical, mental health, social services, vocational/educational services are obtained & coordinated for each consumer.\
- Using Evidence Based Practices ( illness management & recovery, motivational interviewing, & WRAP) provide supportive counseling which assists each consumer in successfully living in community settings.
- Provide Crisis Intervention for consumers.
- Assist consumers in establishing positive interactions with family, neighbors, landlord, etc.
- Assist consumers in becoming familiar with their community & the resources available to them.
- Assist consumers with both medical & mental health needs, providing transportation, as needed, & providing a communication link between health providers & the consumers.
- As a member of the MESH team , the Specialist discusses with the consumer and other Team members what Recovery goals the consumer wants to work on & how to proceed with that goal. The Specialist is responsible for documenting this plan & putting it in the clinical record, in accordance with regulatory time frames.
- The Specialist is responsible for documenting progress &/or regression in the attainment of goals & what interventions are planned. This documentation is done on a monthly basis.
- Assist the consumers with budgeting & financial management.
Position Requirements
- Bachelor's degree from an accredited university in a mental health related discipline.
- Minimum of 2 years experience in a behavioral health setting.
- Ability to be on-call & a flexible work schedule.
- A valid driver's license is required.
