• Conduct comprehensive, ongoing assessments of program clients’ mental illness, substance abuse, physical health, housing status and other relevant needs.
• Complete DLA-20’s every six months on primary case load as required by DMH and BJC/administrative agent.
• Provide direct clinical services to clients on individual and group basis in office and in community settings, make appropriate changes in treatment plans to ensure client’s ability to cope with internal and external stresses and transport clients to appointments as scheduled.
• Refer clients to appropriate community agencies, assisting clients when needed.
• Act as liaison and consult with community agencies and program clients to maintain coordination in treatment process.
• Potentially facilitate stage-wise treatment substance abuse groups with emphasis on facilitating personal growth and development towards recovery and wellness.
• Coordinate with outside inpatient services to provide needed services to clients and establish linkage with outpatient treatment groups and other support networks.
• Collaborate with ACT team coworkers during daily and weekly staff meetings.
• Provide person-centered direct clinical services to clients on individual and group basis in office and in community settings, make appropriate changes in treatment plans to ensure clients’ ability to cope with internal and external stressors.
• Facilitate clients’ treatment planning with an emphasis on fostering personal growth and development towards recovery and wellness.
• Provide on-call crisis intervention in coordination with ACT team coworkers, covering nighttime hours, holidays and weekends.
• Maintain service line and BJC-BH client tracking systems and client files (including treatment plans), document client progress to maintain permanent record of client activity within 24-hour timeframe required by DMH and BJC/administrative agent.
• Responsible for ministry policy regarding work hours, building security and all written personnel standards.
• Ensure the care model by interacting with all other service line programs.
• Provide consultation and training for ACT team regarding substance abuse and addiction.
