Job Overview
Catholic Charities of Southwest Kansas is seeking a compassionate, dedicated Family Support Specialist to join our team. This full-time position is available in either our Dodge City or Great Bend office, with regular travel between locations.
You will help families experiencing or at risk of homelessness by connecting them with housing resources, emergency assistance, and SNAP benefits. Working within our Southwest Kansas Supportive Housing Program, you will provide case management services that help individuals build self-sufficiency and maintain permanent housing. This position also handles Disaster Case Management Services in the event of a disaster in our area.
Responsibilities:
- Assess potential prospects for the Southwest Kansas Supportive Housing Program
- Assist and support those in our housing program to help them become self-sufficient and maintain housing
- Collaborate with community resources to find necessary assistance for those in need of housing
- Coordinate with landlords to ensure appropriate housing for program participants
- Assist community members with Emergency Assistance Applications
- Provide case management services and complete Housing Stability Plans with participants
- Provide information, application assistance, and outreach regarding SNAP benefits
- Complete CoC recommended assessments on those experiencing homelessness
- Complete home visits and outreach activities in the community
- Maintain accurate and up-to-date case notes within 72 hours
- Submit documentation in compliance with Catholic Charities and HUD Policies and Procedures
Qualifications:
- Bachelor's Degree (Preferred)
- Licensed social work
- Case management experience
- Proficient in written and verbal communication
- Ability to work collaboratively with people of diverse backgrounds
- Proficiency in MS Office and TEAMS
- Must have a valid driver's license and good driving record
- Ability to maintain client and agency confidentiality

- Flexible working hours may be required
- Travel between offices is required
